SUMMER CAMP 2024 Week 3! 6/23/2025-6/27/2025 Camp Hours 7:30AM- 6PM
School’s out for Summer! Our Summer Camp program is designed to let kids have a summer to remember filled with games, sports, arts & crafts, movies, field trips and more!
Registration can also be done in person at Wes Crile Park, 1537 Norbert Terrace Deltona, FL. 32738
Make sure you have completed the initial intake registration before completing any camp week registration.
You will be required to register for each camp week separately. Registering for one week does not guarantee you a spot for another week until it is paid.
PROGRAM WAIVER AND RELEASE OF LIABILITY
In consideration of being allowed to participate in any way in (City of Deltona) athletic sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:
The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (City of Deltona) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY COMPLETION OF REGISTRATION, AND AGREE TO WAIVER FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

(UNDER AGE 18 AT THE TIME OF REGISTRATION)
This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasees, and for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liabilities incident to my minor child’s involvement or participation in these programs as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent permitted by law.
PHOTO WAIVER AND RELEASE OF LIABILITY
The City of Deltona (COD) reserves the right to use any photograph/video taken at any event sponsored by COD, without the expressed written permission of those included within the photograph/video. COD may use the photograph/video in publications or other media material produced, used or contracted by COD including but not limited to: brochures, flyers, television, social media websites, etc.
To ensure the privacy of individuals and children, images will not be identified using full names or personal identifying information without written approval from the photographed subject, parent or legal guardian
A person attending a COD event who does not wish to have their image recorded for distribution should make their wishes known to the photographer, and/or the event organizers, and/or contact COD at 2345 Providence Blvd., Deltona, FL 32725, in writing of his/her intentions and include a photograph. COD will use the photo for identification purposes and will hold it in confidence.
By participating in a COD event or by failing to notify COD, in writing, your desire to not have your photograph used by COD, you are agreeing to release, defend, hold harmless and indemnify COD from any and all claims involving the use of your picture or likeness.
Any person or organization not affiliated with COD may not use, copy, alter or modify COD photographs, graphics, videography or other, similar reproductions or recordings without the advance written permission of an authorized designee from COD.
CAMP DISCIPLINE ACKNOWLEDGEMENT
Campers are expected to behave and follow camp rules while they attend camp. Campers may be asked to stay home from camp if discipline problems persist. I understand that I will be notified in advance of discipline problems and that I will not receive a refund for days missed for discipline reasons.
The contact will be automatically added to the class as a registrant if someone drops out from a full class.
